Record Requests

The Phoenix Center maintains electronic and some paper client records. Clients may submit a request to review, copy, or make amendments to their treatment records or to have a copy of their records sent to other specified individuals or organizations.

The Phoenix Center is prohibited under Federal Alcohol and Drug Abuse Confidentiality law, 42 C.F.R. Part 2 from responding to a records request or from confirming or denying that an individual is receiving or has received services from The Phoenix Center without the proper consent.

Clients must complete the Release of Confidential Information Form prior to the request being completed when the information is to be released to anyone other than the client.

When possible, a summary will be provided to the requestor instead of a separate copy of each individual document. 

A fee is charged when copies are made at the request of a client or a third party with which the Agency does not have a Memorandum of Agreement.  Charges include:

  • A $15 administrative fee for each copy of the chart up to 10 pages to cover searching and handling plus the cost of postage.
  • A charge of $.40 for each additional sheet. 


Client payment is required before the copy of the file is released. For third party requests that we do not have a MOA, an invoice is sent along with the copy of the file and is required to be paid upon receipt.

Clients may receive a copy of their Discharge Summary and drug screen results at no cost. Clients will need to provide a picture ID when picking up records. Records may be mailed to the last known address on file.
